Enriched Care

Enriched Care is an innovative new program created to educate, advocate for and coach older adults in their pursuit of successful aging.  The goal of the program is to assist participants in achieving maximum independence based upon the findings of a thorough individual health assessment. 

 

Enriched Care encourages participants to break away from the thought that a decline in health as you age is inevitable, and the best you can do is react to it as it occurs.  We empower people with information about their health and give them the tools to help prevent decline. Research shows that decline is predictable, and early intervention increases your chances of maintaining independence.  We also stress to participants the idea that one area of decline can contribute to decline in other areas, so a whole-person, interdisciplinary approach is needed. 

 

Our wellness approach through the Enriched Care Program is to screen for areas of risk, educate the senior about their risk, and coach and empower them to act in order to help preserve function. The emphasis of the program is health education, and participants benefit from an interdisciplinary team reviewing their health concerns in detail, ensuring participants and their loved ones have a full understanding of their health risks. 

 

The team, which includes a registered nurse, physical therapist, occupational therapist, registered dietitian, chaplain, certified personal trainer, social worker and pharmacist, prepares a Personal Health Plan for each participant, which summarizes needed health and lifestyle changes that have proven results in improving the health of older adults.  We share the results with your personal physician, and strive to complement – not replace – him or her by providing you with the details and thorough information they may not always have the time to explain.

The program was designed by Oakwood Village staff in consultation with Dr. Steven Barczi, Associate Professor at the UW School of Medicine and Public Health, Division of Geriatrics.
